ActionReach Zambia

Faith in action. For every child.

Every child deserves to be recognised, protected, healthy, learning and able to reach their full potential.

Too many children are missed because a birth record is missing, a health or education barrier goes unresolved, a referral is never followed up, or a family has no one to help navigate the next step. ActionReach Zambia works alongside children, caregivers, churches, communities, schools, health facilities and Government to help turn children’s rights into practical progress. We stay with the process, from identifying a child and understanding their needs to coordinating action and verifying what has changed.

The challenge

Too many children are still being missed

A child can be excluded from essential services for reasons that appear small in isolation but become significant barriers in everyday life. A missing birth record can complicate access to services and make a child less visible in systems. A child may miss health or nutrition care because a caregiver cannot afford transport, does not know where to go, or cannot complete the next step. A child may be out of school, living with an unaddressed disability, facing neglect or violence, or growing up in a household under severe pressure.

These challenges rarely occur one at a time. They can reinforce one another, leaving families to navigate systems that are difficult to access and providers who may not know whether a child received the support intended. A referral, an application or a visit is not the same as a completed outcome.

ActionReach focuses on this last mile: the practical work between identifying a need and confirming that a child has received appropriate support and made progress.

Our response

One child. A connected response.

We use the COUNT–GROW–THRIVE model to bring greater continuity and accountability to child-focused support. Each child is considered as an individual, not just a statistic. We identify the barriers affecting the child, agree a practical plan with the child and caregiver, work with responsible service providers, track agreed actions and verify progress. Where appropriate and safe, we follow up after the initial intervention to understand whether the change has lasted.

  1. COUNT

Identify the child, understand their situation and support legal identity where needed.

  2. GROW

Coordinate action across health, nutrition, education and protection, and follow through.

  3. THRIVE

Verify progress, check whether it is sustained and learn how to improve.

Two ways in

Two ways a child can enter support

Entry point 1 · Our signature entry point

Birth Registration & Legal Identity

A birth record helps make a child visible and recognised. ActionReach works with caregivers and relevant authorities to help families understand and navigate birth-registration processes, identify missing documentation and follow the process through to confirmation. Legal identity is our signature entry point because a child who is not recognised in systems can be missed by the services and protections intended for them.

Lifeline

Some children first come to our attention because of an urgent or unmet health, nutrition, education or protection need. Lifeline provides an alternative entry point so that a child is not turned away or delayed simply because they do not have a birth record. We assess the immediate concern, take appropriate safeguarding steps and coordinate with the responsible service providers.

Health & Nutrition

We help children and caregivers overcome barriers to appropriate health and nutrition support by identifying unmet needs, supporting navigation and follow-through with health facilities and relevant providers, and checking whether agreed care was accessed.

Impact at a glance

Our impact, 2020–2025

From 2020 to 2025, ActionReach contributed to reaching children through health and nutrition, education and child protection activities. These years established the community relationships and practical learning that inform the more structured approach being implemented from 2026.

14,102children reached through health and nutrition
3,577children reached through education
3,465children reached through child protection
21,144total area-based counts across the three areas, 2020–2025

Measurement note: the 21,144 total is the sum of the three programme-area counts. It represents children reached by area of support and should not be described as a deduplicated count of unique children unless individual records confirm that children have been counted only once.

The next chapter

The next chapter: 2026 onwards

In 2026, ActionReach began a new phase: moving from emergency response and community reach towards structured last-mile delivery. The focus is not simply to reach more children, but to strengthen the follow-through around each child. We are building a consistent process for individual records, child-specific plans, named actions, outcome verification and follow-up.

2020–2025

Emergency response and community reach

21,144 programme-area counts across three areas; church volunteers identified children isolated at home; support was delivered child by child and counted by area.

2026–2031

Structured last-mile delivery

Two entry points; one record and one plan per child; outcomes verified before case closure and checked at three and six months; ambition to reach 40,000 children directly, each counted once.

Our commitment

Every child counted. Every barrier addressed. Every outcome verified.
